Similar situation: the 16 year old coming in for a sick visit, and when you ask her why she is coming to see the doctor, she turns to mommy and says, "I don't know, ask her".
The patient didn't have one...but his intern did."I am calling you because Mr. Jones has a scary looking EKG."in my note my first sentence, "Reason for consult: "Scary looking EKG."My diagnosis...VT storm.It actually was in the scary range.
